Pathway Students (4th year UG)

  • This preliminary Program of Study form must be completed in collaboration with major advisor and approved by the Undergraduate and Graduate Coordinators, prior to the start of Fall semester, 4th year.
  • Advisory Committee Form
    This form is due during the Spring semester of your 4th year

Graduate Teaching Assistant Forms

First Year Forms

  • Advisory Committee Form 
    You must submit an Advisory Committee Form during your first year of graduate work. This form should be filed before the end of prior to your 2nd semester review.  
  • Second Semester Review
    This is the preliminary Program of Study form to include in your 2nd Semester Review Packet. PhD students must submit the 2nd semester review form directly following the meeting.   


Second Year Forms 

  • Program of Study  
    PhD students must submit a Program of Study Form (G138) for approval by the dean of Graduate School. It must be approved by the time the notification of the preliminary oral comprehensive examination is given. The form will be reviewed by the Grad School and they may request revisions. Please make the initial submission with this in mind. 
  • Comprehensive Exam Announcement 
    This form will be submitted by the Graduate Coordinator Assistant. Please note that it must be submitted at least 2 weeks prior to the scheduled oral exam. Inform the GCA of the time and date of the exam with sufficient time for the form to be submitted. 

Third Year Forms 

  • Prospectus Defense Approval Form 
    Students are responsible for ensuring that committee members sign the form and for submitting it to the Graduate Coordinator Assistant. Please also submit a pdf of your prospectus.  
  • Application for Admission to Candidacy 
    This form (G162) must be signed by your Major Professor and the Graduate Coordinator and filed with the Graduate School. You need to initiate this form through GradStatus. 
  • Research Approval Form 
    This form must be completed and submitted prior to leaving for fieldwork or initiating fieldwork.

Dissertation and Graduation Forms 

Dates and deadlines for all these forms can be found on the Graduate School Website. 

  • Apply to Graduate
    This deadline is usually very early in the semester. If you plan on graduating or think you have a very good chance of graduating, you must submit this form at the beginning of that semester. If you don't defend or graduate in that semester, there is no penalty. You will need to reapply to graduate in the following semester.
  • Format Check 
    Before defending your doctoral dissertation, you must submit one complete copy of your thesis or dissertation for a format check with Graduate School.  
  • Electronic Thesis and Dissertation (ETD) Submission Approval Form 
    Before scheduling your doctoral dissertation defense, you must submit the final ETD Approval Form and a corrected copy of thesis/dissertation.  
  • Approval Form for Doctoral Dissertation and Final Oral Examination 
    This form (G164) must be submitted a week prior to your doctoral dissertation defense. You need to initiate this form through GradStatus.
  • Dissertation Defense Announcement 
    This form will be submitted by the Graduate Coordinator Assistant. Please note that it must be submitted at least 2 weeks prior to the scheduled oral defense. Inform the GCA of the time and date of the exam with sufficient time for the form to be submitted.
